Position Summary
The Nurse Practitioner provides professional comprehensive primary medical care to ambulatory LCHC patients in collaboration with health center staff physicians.
Principal Duties and Responsibilities
1. Obtains health history and performs a physical exam to determine a diagnosis and treatment plan in accordance with established standards, procedures, and practices. Requisitions lab tests and x-rays, writes prescriptions, and makes referrals, as appropriate.
2. Assesses, treats, and manages specific medical illnesses and conditions according to medical regime, utilizing established protocols.
3. Consults and works collaboratively with physicians and other health care providers to develop effective treatment plans. Provides direction to other personnel and reviews medical records to verify that each patient’s care plan and medical needs are being met.
4. Provides referrals for hospitalization, tests, therapies, or specialty consultations; makes hospital rounds as indicated.
5. Provides follow-up care for patients. Ensures continuity of patient and family care by exchanging information and cooperating with other community health/social service agencies, hospitals, and health care providers.
6. Counsels patients and/or significant others concerning physical and developmental well being, defining each individual’s needs and problems. Identifies community health resources, and guides patients and/or significant others in their use.
7. Develops and implements plans for teaching and counseling individuals, families, and groups in various settings for the prevention of disease and health maintenance.
8. Attends staff meetings and provider meetings, and maintains involvement with LCHC committees to ensure quality of care for patients; participates in development and/or revisions of specific protocols; participates in peer review, performance evaluations, and the review of improvement data to assess and advance quality improvement measures.
9. May perform other duties as required.
Compliance and documentation
Confirm patient identity according to current procedure before rendering any service.
Thoroughly, accurately, and promptly document all service rendered in the electronic medical record for every patient.
Perform all tests, procedures, and other services according to the policy and procedure of LCHC, notify the appropriate supervisor in a timely manner of instrument or procedural problems.
Be knowledgeable of and in compliance with CLIA, OSHA, and JCAHO regulations for safety, infection control, equipment operation, and other applicable areas.
5 Maintain confidentiality of patient health information in accordance with HIPAA and other applicable regulations.
6 Run instrument and test controls, monitor and replace needle and syringe containers, maintain and sterilize equipment and instruments, monitor and maintain inventories, and perform other tasks as needed to maintain a safe and well functioning work environment
7 Participate in proficiency testing as required by LCHC or departmental policy to ensure that proper standards of care are being maintained
Required Skills and Abilities
Ability to work independently and make clinical assessments.
Required Knowledge, Experience, Licensure or Registration
Education: Graduate of an accredited Nurse Practitioner program; Master’s Degree preferred. Additional training may be required for specialty area.
Experience: Qualified by education, training, or experience to work with the adult and/or geriatric patient population as specialty assignment dictates. Demonstrated ability to work independently and make clinical assessments. May require additional experience in specialty area.
Licenses or Certifications: Current licensure or permit as RN in Illinois. Nurse Practitioner certification or registry eligible required. Additional certification in specialty area may be required. Eligible for Certificate of Fitness (prescriptive privileges) in Illinois required.
